One pulse fitting from the crank case, one fuel line fitting...where is the other fuel line fitting? We need one in and one out.
That thing looks like it was a factory fitting. I think California used it as an emission sniffing thing to determine if the exhaust had any more cow manure in the exhaust than it had in the intake. Excess manure scent is the exhaust has been linked to a higher risk of cancer in California.

Granted, we can't see the underside, which might lead us more in the manure direction.

Does that empty fitting go to vacuum?
The yellow one appears to be on vacuum from side cover. The empty one on top is so clean I'm thinking it's the fuel out. Maybe the fuel in fitting is just not visible? Maybe off a boat or sled because the diaphragm housing looks too big for a 140.

Ok, so if it is a fuel pump, why? Wouldn’t it be gravity fed? Or what application wouldnt be ? I dunno

I'm going to guess leaning off the back of a john boat with a prop on a really long shaft. I've read this was an early popular use for these GX OHVs in the Japanese market. Where did you get it? Near water?

Vacuum from side cover?
My side cover has crankcase pressure coming from it, not vacuum. Vacuum comes from intake manifold.

I figured it was an actual question, not another item to be disscussed. It's two bolts/nuts. (shrug) < Did you note that shrug? Are we supposed to guess? I mean it looks a LOT like a muffler bearing. I've never actually seen one, so maybe this is one.

Ford used a sensor to measure the differential pressure in the EGR valve in the eighties and nineties, called a DPFE sensor. It looked like that thing.
HOWEVER, I do believe it as a fuel pump in this instance. That Honda does not have a fuel tank mounted on top, so it must have had a remote fuel tank. A remote fuel tank needs a fuel pump, so, my conclusion is it must be a fuel pump.
