If you look at the picture, as soon as the load lifts up, the right side of the U bolt will slide off the pin since nothing is holding it on. The u bolt is suppose to straddle the angle iron followed by the pin and you would also want to use a nut on the right side of the pin.

Which part ... the the hook bolted on one side or the 3/4" screw in a 2x4?

Looks like they are trying to hook to the bucket of a back hoe?
In that set up I bet the wood gives before the hook and pin ... unless there is a steel plate I cant see under the 2x4.
 
Look again, Chris. The right end of the pin is threaded into the clevis. That does not forgive the obvious error of incorrect fastening. The clevis should be straddling the angle, with the pin through it all. On top of that, as soon as the load is applied, that angle will be placing a side stress on the bolt. In no way is this rigged anywhere correctly. What we do not know is what the overall load is, as the pic doesn't show that.

You make a good point that the U bolt may be threaded on the right side, can't tell from the picture. Like you said, there is a lot in the picture that is troubling, as well as what we do not see that may be even more troubling.
