Is your modem actually going offline itself or is your computer locking up / losing connection? Check the lights on your modem to see which ones are on now and look again when it happens next.
I would call your provider, sounds like it could be your modem. I'm a system tech for a communication company... not sure how their system works, but there's only a couple of things it could be. Could be a signal issue too, but probably the modem.
